Cirrhosis

The liver is responsible for neutralizing the blood of toxins, germs and bacteria as well as producing immune agent to contrl infections. Bile, critical to the absorption of fats and fat-soluble vitamins is made by the liver. Cirrhosis is a condition where the liver is affected by irreversibble scar tissue leading to its damage and consequent failure. Blood flow to the liver is then affected. Symptoms of cirrhosis range from exhaustion and fatigue to weight loss and abdominal pain. A person suffering from liver cirrhosis may experience abdominal pain and loss of appetite. There are noticeable red spider veins under the skin and the skin and eyes may turn yellow. There is decreased interest in sex and edema (swelling on hands and legs) might be noticed. A person suffering from cirrhosis and damaged liver may notice an increased tendency to bruise and bleed easily. Intense itching is felt on account of the bile products being deposited in the skin. Gallstones may develop as a result of inadequate bile reaching the gallbladder. There might be a buildup of toxins in the brain bringing about bouts of unresponsiveness and forgetfulness. Chronic hepatitis - Hepatitis C virus can lead to severe inflammation and damage of the liver, thereby causing cirrhosis. Hepatitis B is one of the most common causes of liver inflammation in many of the developing nations.
Blocked bile ducts - In such a condition, the bile is unable to travel out of the liver and instead ends up damaging liver tissue. This can be a congentinal defect in some infants.


Cirrhosis of the liver can cause many other abnormalities. It can leads to elevated levels of triglycerides, cholesterol and sugar. Diabetes mellitus is a common fallout. There might be a fall in platelet count and GI bleeding. In severe cases of cirrhosis, there can be an immune system dysfunction or even brain swelling and later coma. The liver of an affected person will feel be larger and harder to touch. A liver scan or ultrasound can help detection of cirrhosis. A liver biopsy is sometimes resorted to. Damage to the liver due to cirrhosis cannot be reversed but further complications can be reduced with the right treatment. Cirrhosis caused by excess alcohol consumption needs lifestyle changes such as avoiding alcohol and following a nutritious diet. Low-sodium diet can help drain excess fluid-buildup within the body. Chronic viral hepatitis B and C are treated with prednisone and azathioprine. Any bacterial infection is treated with appropriate antibiotics. Liver transplantation surgery is done on cases where the liver is not capable of functioning. With the help of modern drugs such as cyclosporine and tacrolimus, the success of liver transplantation surgery has risen manyfold.

Gallstones

While gallstones usually form in the gallbladder; they can be found in bile; in the intrahepatic, hepatic, common bile, and cystic ducts. Gallstones ofen do not cause any symptoms. They are often discovered when having a routine x-ray, abdominal surgery, or abdominal ultrasound. When gallstones move from the gallbladder into the cystic duct of common bile duct, it can lead to severe cramping pain in the upper right abdomen. This can last from a few minutes to a few hours. Cholesterol gallstones are more often noticed in women who are obese, on HRT or with elevated blood triglyceride. Black pigment gallstones are usually formed when there is increased destruction of RBC. Others who are likely to suffer gallstones are those suffering liver cirrhosis and biliary tract infections or sickle cell anemia. Gallstones can form when the gallbladder does not empty bile properly. Blood tests to check bilirubin, liver function test and pancreatic enzymes are often prescribed. In cases of severe painful episodes, cholecystectomy is performed.

Pancreatic Cancer

Pancreas lies deep within the abdomen and secrete enzymes that are vital to digestion. Hormones secreted by the pancreas aid in maintaining sugar levels in the body. Pancreatic cancer is a condition where there is a malignant tumor, mostly found in the exocrine portion of the pancreas. Pancreatic cancer is not easily detectable in the early stages. Besides, it spreads rapidly and can metastasize to other organs such as bile duct, duodenum (small intestine), spleen, colon, adrenal gland, kidney, or vertebra.


Typical symptoms associated with pancreatic cancer are loss of appetite, nausea, vomiting, abdominal pain and weight loss. The patient is likely to suffer from back pain and complain of tiredness and fatigue. Pancreatic cancer may result in blood clots and liver dysfunction. The abdominal pain associated with pancreatic cancer is likely to radiate to the upper and middle back. There may be yellowing of the skin since most persons suffering from cancer of the pancreas tend to develop jaundice. In advanced stages of pancreatic cancer, there is considerable accummulation of bile acids in the skin leading to severe itching.


Some peope have a heriditary genetic predisposition to cancer while there are certain factors that increase the likelihood of developing pancreatic cancer.

Blood test for serum bilirubin will show high bile pigment. Abdominal ultrasound and abdominal MRI aids in diagnosing the extent of tumor in the pancreas. ERCP (Endoscopic retrograde cholangiopancretography) is a procedure that allows a physician to observe the common bile duct and pancreatic ducts. A pancreatic biopsy can help understand the kind of tumor in the pancreas. Pancreaticoduodenectomy (Whipple procedure) involves surgical removal of the pancreatic tumor. This surgical procedure is conducted on patients whose tumors are located in the head of the pancreas or regions adjacent to it. Surgery may involve remove a section or the entire pancreas. A combination of radiation therapy and chemotherapy is given to those patients whose tumor is not removed.
